2023-2024 Sports Team Schedules

Parents need to sign off on all the Athletic Forms in their Parent Powerschool Account
High school sports permission and emergency authorization forms are all available in Powerschool.
Once you have completed the online forms they will be submitted directly to the Athletic Director.
To begin the process, log into your Powerschool parent portal account and on the left navigation menu choose HS Sport Signoff.
Prior to participating in tryouts, practices or games, the following forms must be completed and be on file with the school:
Sports Physical
Parent Permission
Concussion Annual Review
Sudden Cardiac Arrest Annual Review
Covid Consent Waiver
The Parent Permission, Concussion Annual Review, Sudden Cardiac Arrest Annual Review and the Covid waiver can be completed electronically via PowerSchool in the “Forms” section.
If needed, copies of all of the Athletic forms are available below.

HEALTH PHYSICALS INFORMATION
Physicals should be either emailed, mailed or dropped off to the school to the attention of:

Please make a copy of the completed physical for your files in case another copy is needed later in the school year.
*All physicals are valid for 13 months
*Please note that although the four signoff forms can be completed electronically, the Health physical packet must still be submitted in paper form to the nurse’s office after the physician has completed it.
Cancellations
Go to www.casciac.org
Click on E-mail Updates (to the left of the GO button)
Click on E-Mail & SMS Updates (use either the SMS Icon at the top of the page or the CIAC Quick links at the bottom of the page)
Click on E-Mail Updates
Choose “Click here to subscribe to this service or change your subscription settings.”
At the bottom of the page follow the steps to Create a New Account and click “Create Account.”
Using the “Add a High School Update for a CIAC Sport” select Foran High School and the sports and levels you would like to receive updates on and click the “Add Selected High School Update” bar. You can only add one sport at a time, but you can add as many as you want.

Those dates will conflict with CIAC Tournaments. Please plan accordingly
